Complicated forms

Hello, everyone. I have a bit complicated question (at least for me). Maybe someone could help and answer how to make that kind of form like on this website

when user choose some options on every step and finally all of the information is sent to the back-end (like CRM system or email at least)

UPD Thx for your answers, guys. I was hoping that it’s possible to do with Blocs Tools. But it seems that functionality of the app doesn’t let you do this from box. Sad.
