yes, very easy
use the standart form and drop it in your bloc
choose 2 or 3 column row and drop it IN your form
now you can drag an input field in the new column
or ad extra fields there

@sandy, i tried to do it your way before posting on forum, but something goes wrong and I made a big mess.
