Unwanted items appearing in menu

I can’t seem to remove a menu item. It’s not listed in the Menu Manager, and unchecked for ‘Use in Primary Menu’, but it appears in the menu anyway.
Any ideas how to remove it?

I sorted it. Spotted the refresh icon and clicked on that.
